Transcript Request

Please review the following information before requesting your transcript.

For those who need Education Verifications, please do not use the Davis University Transcript Request form. Please call 419-473-2700 or email the Academic Office at david.flowers@davisuniversity.edu.

Before you begin filling out your transcript request form, you will need to choose whether to request electronic or hard copies of your transcript.

Once you selected your preferred method of sending your transcripts, you will begin filling out all the required information to help Davis University provide a speedy and smooth process. The following criteria will need to be provided,

  • First Name
  • Last Name while in attendance
  • Last 4 Digits of SSN (If Applicable)
  • Date of Birth
  • Phone Number
  • Email Address
  • Street Address (US or International)
  • Year(s) of Attendance (Ex. 2018-2020)
  • Date of Graduation
  • Purpose of Transfer
  • Receiver(s) Information Name (University or Individual’s Name/Email)
  • Receiver(s) Address Information (US or International)

E-Transcript (Electronic Transcript)

If you choose to send your transcript to another college or university, please confirm with your incoming institution that they can receive an e-transfer of your transcript from our Registrar’s Office.

Hard Copy Transcript (Standard Mail Transcript)

If you choose to send your transcript to another college or university via hard copy, please make sure to have the correct Name and address of the receiving institution within the transcript request form to avoid delays.

Processing

Once Davis receives the transcript request form and confirms that the correct payment amount has been received, Davis University will begin processing the transcript.

Please expect 3-5 business days for all transcript requests to be filled. If you require an expedited transcript request, please email transcript@davisuniversity.edu.

Cost

The first transcript fee is $30, with each additional transcript costing $20. With a maximum of 10 each time.

(Example: First transcript is $30, the second $50, etc.)

You will locate links to make your transcript request payment once you submit a successful transcript request form. You will be taken to a ‘Successful Submission’ page with two links. One link will be for credit or debit card payments, and the other will be for bank wire transfers.
